Pre-registration System!
We HIGHLY RECOMMEND you fill out your registration forms before coming into the office! No one likes to fill out forms at the doctor's office, but this way its done before you step foot in our office. It will save you time during you busy day.
Click on "Complete an Online Welcome Form" Icon on the left side of this screen. It will guide you in providing your personal, medical history and eye history information. When you do this online, you won't have to fill out forms at your appointment.
Once you have completed the forms, click "Submit", and it will come straight to us. The entire process is done via a secure connection (like a bank), so all your information is safe.
Once you have submitted your registration electronically, we still recommend you print out a copy of your registration to bring with you to your appointment. Like a fax or an e-mail, its better to have a back up!
Online registration is just one more way we try to save you time and make things easier!
If you would like to request an appointment via email:
Click on the "Send us an email" link below and provide:
- The patient's name (and parent's if a minor)
- Your best day time phone number we can call you back at
- The type of appointment you need. PLEASE be as specific as you can (diabetic eye exam, glaucoma evaluation, contact lens check, frames adjustment, etc.)
- Provide us with BOTH your medical insurance and vision plan names
- What day and time you would prefer, so we can check availability
A staff member will contact you as soon as possible. |
